Overview
Tirzepatide is a 39-amino acid synthetic peptide with a C20 fatty diacid modification enabling once-weekly dosing. It was designed as a "twincretin" β a single molecule that simultaneously activates both GLP-1 and GIP receptors with approximately equal potency. This dual action proved more effective than GLP-1 agonism alone in clinical trials, challenging earlier assumptions that GIPR activation might blunt GLP-1 weight loss effects.
The SURMOUNT-1 trial demonstrated 20.9% mean weight reduction at 72 weeks with the 15mg dose. In the SURPASS program for T2D, tirzepatide outperformed semaglutide 1mg for both HbA1c reduction and weight loss. Ongoing trials are studying tirzepatide in heart failure, sleep apnea, MASH, and kidney disease.
Mechanism of Action
Tirzepatide's GLP-1R component drives appetite suppression, delayed gastric emptying, and glucose-dependent insulin secretion. Its GIP receptor activity may enhance adipocyte sensitivity to GLP-1 effects, increase adipokine secretion, and independently reduce appetite through central mechanisms. The combination produces effects greater than either agonist alone.
